Video Information Form

In order to have a Live Virtual Visit via Zoom or Skype or a Personalized Video Message Santa will need some information.

Please complete the following form. Once completed, you will be directed to fill out a separate form for each recipient on the Virtual Visit or for the Personalized Video Message.

After all the form submissions are completed you will be contacted to arrange a time, payment and finalize any remaining details.
